(*) Your name & phone number are required fields, so that Brisbane Obesity Clinic can contact you to confirm your appointment.

When you come for your appointment please remember to bring the following:

• Referral from your General Practitioner
• Medicare or Veteran Affairs card
• Private insurance information if you have insurance
• Any scans, x-rays or blood results or previous doctors letters if you have these

To cancel an appointment:

Please phone the office during business hours.  It would be appreciated if you could cancel an appointment at least one day ahead so that we can allocated your appointment time to another patient on our waiting lists.
